Healthy New York

The Health Care Reform Act of 2000 introduced "Healthy New York", a program that promotes and provides affordable insurance coverage to eligible small businesses not currently offering health insurance coverage to their employees. The program is also available to eligible uninsured Individuals and sole proprietors.

Syracuse Area (Broome, Cayuga, Onondaga, Oswego, Tioga counties)

Metro Area (Bronx, Duchess, Kings, New York, Nassau, Orange, Putnam, Queens, Richmond, Rockland, Suffolk, Sullivan, Ulster and Westchester counties)

To apply for the Healthy New York plan, please download and mail the completed Application and supporting documentation along with the first monthly premium payment to:
